Care coordination
Let our team of care coordinators take care of your patients
UCM’s Care Coordinators work alongside our medical and mental health providers to ensure that care is delivered seamlessly to the patient and that all follow-ups are handled in a timely manner.
All of our Care Coordinators receive extensive hands-on training, and have prior medical, case management, or customer service experience.
Care Coordinators help with:
- Prescriptions
- School or work notes
- Finding in-network labs, facilities, and providers
- Ordering durable medical equipment
- Making follow-up appointments with a UCM provider
- Coordinating follow-up appointments with a non-UCM provider
- Connecting to community resources
- Coordinating care between providers, like PCPs, mental health providers, and specialists
